Review Incident By HOD

  1. Login as “Plant HOD” . Go to My Action | For Your Review tab.
  2. All the reported incident details will be populated in a tabular manner.
  3. Click Subject, Section, Action to be Taken, Assign Date, Target Date and Status on header on the table to view the incident in ascending/descending order.
  4. Click on “Hyperlinked Subject” in Subject column to review the incident which will open in different tab. (Image 2)
  5. By default, all prefilled data will be automatically displayed. Unit, Project/Plant, Area, Shift, Event Date & Time will be disabled.
  6. Select Authorization by selecting any one option from the drop-down list.
    1. Send for Investigation: Select this option if the reported incident is found valid by adding investigation team. Click Submit button to approve the incident report and the investigation team will be notified.
    2. Reconsider: Select this option if any field is to be modified/updated for more clarity.
  7. In case investigation is required, add investigation team members and mark one person as a team leader.
  8. Click Submit button to save the details and proceed to the next action.

Note :

  • Reviewer cannot edit the report contents
  • The fields marked with * are mandatory to be filled.
